Names the unit rather than the profession. ICU nurses generally do not consider their job the same job as a ward nurse, which is exactly why this specific version works.
Trade and profession shirts are specific on purpose. Someone who says they are an intensive care nurse will not accept a near-miss title, and the closely related jobs each have their own version — that precision is the whole point of the joke.
